Position Summary: This position supports the mission of Wesley Medical Center in optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. RN Case Manager facilitates the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, and admission status and resource management while ensuring the best possible outcomes within a fiscally responsible timeframe. RN Case Manager identifies potential barriers to throughput and quality outcomes and facilitates appropriate discharge plans in conjunction with social workers. RN Case Manager is responsible for working during varied shift schedules, actively participates in outstanding customer service and accepts responsibility for maintaining relationships that are equally respectful to all. Daily responsibilities may include discharge planning activities which begin at the time of (or prior to) admission and are reassessed throughout the course of hospitalization. Assesses cost-effective treatment options which maximize patient outcomes and meet patient/family needs Responsible for assessing the psychological, social, cultural and or financial needs that impact patients at high risk for successful transitions on dismissal Assesses relevant clinical and age related information to assist in the development and implementation of timely treatment and discharge plans Tracks and trends barriers to care; makes recommendations and develops action plans to improve processes and systems Maintains rapport and effective communication with the healthcare team and patient/family so the plan of care can be discussed objectively, problems identified, recommendations and adjustments made to the plan as needed Integrates results of performance improvement and case management activities to improves patient outcomes, utilization of hospital resources and length of stay Fosters the patient/family's decision making and autonomy and act as a patient/family advocate Demonstrates the ability to manage throughput, assessing patients continuum of care Acts as a liaison through effective and professional communications between and with physicians, patient/family, hospital staff and outside agencies Applies knowledge of state and federal laws, professional practice standards and other regulatory guidelines Intervenes to resolve system problems that influence billing. Qualifications Required Registration, Licensure or Certification Current licensure as a Registered Nurse by the Kansas State Board of Nursing Current Basic Life Support (BLS) verification course, as specified in policy Required Education or Experience Bachelor's prepared RN with current state licensure. In lieu of BSN, RN must have 5 years case management experience and complete a bachelor ' s of science in nursing within 2 years of hire date. 5 years of nursing experience with 2 years critical care or 3 years nursing experience Preferred Education or Experience 2 years case management experience preferred Recent hospital social work or case management experience Required Knowledge and Skills Ability to apply critical thinking skills Demonstrates self-direction and motivation Demonstrates highly developed written and oral communication skills Demonstrates assertive interpersonal skills Practices positive group dynamics and relationship building skills Demonstrates accomplished presentation skills Working knowledge of performance improvement Demonstrates basic computer skills Demonstrates an ability to affect change
